Please go to your Genesis Parent Portal to complete the Spring season registration. The registration link can be found in the "Forms" section, not the "Athletics" section.

Parents can access the "Athletics" section to confirm once an athlete has been cleared.

Four action items need to occur for sports clearance:

• Online registration
• Medical clearance from the school nurse
• ImPact Concussion testing (HIGH SCHOOL STUDENTS ONLY) clearance by the Athletic Trainer (all 9th graders and new students; every other year for all others unless post-concussion)
• Paid Activity fee (click here for the high school activity fee letter - click here for the middle school activity fee letter). The Activity fee is a one-time payment for the school year that covers all activities and sports. Students who will only do crew do not need to pay the HSD activity fee; they only need to pay the dues for the Haddonfield Crew Club.

Registration must be completed by February 28 to avoid delays in being cleared for participation.

FOR STUDENTS WHO NEED A NEW PHYSICAL TO PARTICIPATE IN THE UPCOMING SEASON: An original copy of the LAST PAGE of the Preparticipation Physical Evaluation Medical Eligibility Form MUST be dropped off or mailed to the attention of the high school or middle school nurses, depending on the grade of the student. Please do not email the form. (The Preparticipation Physical Evaluation Form should be maintained by the healthcare provider completing the physical exam (medical home). Although not required, you may ask your physician to keep a copy in the school's health office.

The medical eligibility form (last page) is the only form that MUST be submitted to the school. The physical exam must be completed by a healthcare provider who is a licensed physician, advanced practice nurse, or physician assistant who has completed the Student-Athlete Cardiac Assessment Professional Development module hosted by the New Jersey Department of Education.) Please make sure you keep a copy for your records!

FOR STUDENTS WHO HAVE A VALID PHYSICAL ON FILE WITH THE SCHOOL NURSE AND DOES NOT EXPIRE PRIOR TO THE START OF THE FALL SEASON: Complete online registration, which includes the Health History Update Questionnaire. Responses must be for any health issues that have occurred since the last/current sports physical!
