Sports forms for NEW Physical

FOR STUDENTS WHO NEED A NEW PHYSICAL IN ORDER TO PARTICIPATE IN THE UPCOMING SEASON: An original copy of the LAST PAGE of the Preparticipation Physical Evaluation Medical Eligibility Form MUST be dropped off or mailed to the attention of the high school or middle school nurses depending on the grade of the student. Please do not email the form. (NEW FOR 2024: The Preparticipation Physical Evaluation Form form should be maintained by the healthcare provider completing the physical exam (medical home). Although not required, you may ask your physician for a copy to be kept in the health office at the school.

The medical eligibility form (last page) is the only form that MUST be submitted to the school. The physical exam must be completed by a healthcare provider who is a licensed physician, advanced practice nurse or physician assistant who has completed the Student -Athlete Cardiac Assessment Professional Development module hosted by the New Jersey Department of Education.) Please make sure you keep a copy for your records!

FOR STUDENTS WHO HAVE A VALID PHYSICAL ON FILE WITH THE SCHOOL NURSE AND IT DOES NOT EXPIRE PRIOR TO THE START OF THE FALL SEASON: Complete online registration which includes the Health History Update Questionnaire. Responses must be for any health issues that have occurred since the last/current sports physical!
